The purpose of this course is to provide clinicians with a broader understanding of documentation and its relationship to the standards of practice governed by regulatory bodies. Through presented materials, discussion, and written practice, clinicians will learn how to document for medical necessity and acquire strategies for maintaining client records that meet insurance requirements and uphold professional standards.

Who Should Attend? For all levels of Clinicians, from Graduate Students to Seasoned Clinicians; this important topic should not be missed.

By the end of this workshop, participants will be able to:

  • Understand the definition of medical necessity and how to assess medical necessity for each level of care, specifically outpatient.
  • Identify the necessary components of a mental health assessment, a treatment plan, a progress note, and a discharge summary.
  • Familiarize oneself with how to apply the Golden Thread Process to documenting.
  • Understand the documentation requirements and criteria set forth by insurance companies.
  • Become proficient in how to write clear, concise, and defensible notes through the 3 lenses (ethical, clinical, and legal).
  • Recognize and avoid common documentation errors.
  • Explore how to pass insurance audits and protect their income.
